HealthGap is not an insurance plan.

HealthGap is a medical membership to the Urgent Care of Omaha clinics for services provided solely in these clinics.

This membership requires a 12-month agreement. Monthly membership fees must be paid current in order to receive services from Urgent Care of Omaha.

A $10.00 payment is due at time of visit, per person.

Services provided at the urgent care under the HealthGap Membership program are not billable to insurance.

This membership may be cancelled with 30-day written notice, sent by registered mail, to: Urgent Care of Omaha, HealthGap Cancellation Department, 3830 N 167th Ct, Omaha, NE 68116.

If the membership is cancelled within the first 30 days, via registered letter, after receiving plan documents, HealthGap will refund 100% of all periodic monthly membership fees that have been paid.

This membership will remain in effect for 12 months, unless cancelled prior, by sending a 30-day notice via registered mail.

Any concerns or complaints in regards to services received under the Membership program, should be addressed in writing to: Urgent Care of Omaha, HealthGap Administration, 3830 N 167th Ct, Omaha NE 68116.

All concerns or complaints will be reviewed by the management and medical director for Urgent Care of Omaha, LLC.

If you feel your concerns or complaints are not resolved, you may mail an official complaint to The Nebraska Department of Insurance. Download the form here.
